AI Will Always Give You an Answer. The Question Is Whether It Has the Full Picture.

Think about how you make a hard decision: you pull together everything relevant – the context, the environment, the people involved, the history. You build a picture, and then you make your best judgment.
If you know you’re missing something, you say so. The decision comes with a qualifier. That intellectual honesty is what separates a good decision-maker from a dangerous one.
AI does not do that. It will always give you an answer. Complete picture or not, it reasons over whatever data it has and produces an output. That answer is not inherently wrong, but its quality is entirely dependent on what went into it. And right now, most security AI is working from an incomplete picture of the environment it is supposed to be protecting.
That gap is where trust breaks down, and when trust is gone, nothing gets acted on.
The problem is missing context, not the AI.
In the largest enterprise environments we work with, hundreds of millions of findings a day is not unusual. Volume has never been the hard part. The hard part is connection – the relationships between data points that turn a pile of observations into an accurate picture of actual risk.
In 2025, roughly 1 to 3 percent of published CVEs were exploited in the wild. That number is not static. As AI accelerates exploit development and lowers the barrier to weaponization, that percentage will rise. Google's 2026 threat research already documents adversarial AI autonomously discovering and exploiting vulnerabilities faster than human defenders can patch them. More importantly, the time it takes for a vulnerability to move from disclosure to active exploitation is shrinking fast. The distinction between what is exploitable and what is not is becoming harder to determine quickly. The problem was never surfacing findings. It was always figuring out which ones actually mattered in your environment, right now, before someone else figures it out first.
That question does not have a generic answer. A vulnerability that is critical in one environment is noise in another. Whether something matters depends on whether the asset is reachable, whether it sits on a path to something the business actually cares about, whether compensating controls already exist, whether an active campaign is targeting that specific combination today. Same CVE. Completely different answer depending on who you are.
A platform reasoning from disconnected data will still give you an answer, it just will not be the right answer for your environment.
Same goal. Better execution capacity.
RBVM, CTEM, exposure management… the category names keep changing, but the underlying problem is the same.
It has always been this: identify the vulnerabilities most likely to be exploited in your specific environment and eliminate them before someone does. We did not suddenly discover that risk-based prioritization was a good idea. Security leaders have been asking for it for two decades. What we lacked was the execution capacity to deliver it with precision.
So programs defaulted to coarse-grained rankings and volume. Close as many findings as possible and assume coverage would catch the exploitable ones in the process. That was not the wrong strategy. It was the best available given the tools.
The goal did not change; our ability to finally deliver on it at the precision and scale it always required did. But only if the AI has the full picture to work from.
AI changes the calculus. We can now take the findings we have always been able to surface, correlate them across your actual environment, enrich them with attack chain analysis, reachability modeling, and real-time threat intelligence, and surface the specific exposures that represent genuine risk to your organization. Not to a benchmark enterprise, to yours.
The goal did not change; our ability to finally deliver on it at the precision and scale it always required did. But only if the AI has the full picture to work from.
Incomplete data produces answers that look right but cannot be trusted.
When a skilled analyst works with limited information, they tell you. The recommendation comes with a qualifier. You know what they do and do not know, and you weigh the decision accordingly.
AI does not hedge that way. It delivers a recommendation with the same apparent certainty whether it was reasoning from 40 percent of the relevant context or 100 percent. The risk score looks the same, as does the priority ranking. The ticket gets created and routed either way.
Think about the standard we hold in every other high-stakes domain: A doctor without your medical history and lab test results can still examine you and give you a diagnosis. The reasoning is sound, but you would not accept that as the basis for a treatment plan. You would get the lab tests done first, share your history, give the full picture – because the quality of the decision is directly tied to the completeness of the information behind it.
We accept that standard in medicine, in law, in finance. Security should be no different. If the AI reasoning over your exposure data does not have a complete, correlated picture of your specific environment, the recommendation it produces is the diagnosis without the lab tests. Possibly right, but not trustworthy, and not something you can defend.
Now add agents. The stakes get higher.
When AI is making recommendations a human can review, bad underlying data is a quality problem. When AI is taking action autonomously, it becomes an operational risk problem.
We’re moving fast toward agents that re-prioritize findings in real time, create and route remediation tickets, sequence campaigns across teams, and eventually make control changes autonomously. The speed case is real. The median time to exploit a vulnerability is now under 5 days. The average time to remediate a critical vulnerability still exceeds 60 days (Edgescan, 2025 Vulnerability Statistics Report). That gap is where most breaches happen. Human triage cycles cannot close it alone.
But when an AI agent works with incomplete context, it does not make one bad call – it makes thousands of them, simultaneously, at machine speed, without any signal that something is wrong. A de-prioritized finding that was actually critical. A remediation ticket routed to the wrong team while the window closes. A compensating control evaluated without knowing it had already been bypassed. By the time you realize the underlying data was incomplete, the actions may already be irreversible.
Speed without accuracy just means you get to the wrong answer faster.
Boards are already asking AI accountability questions in financial services and healthcare. Security is next. Can you explain the reasoning behind every AI-driven action? What data drove it? What did the agent know, and what did it not know? If the data foundation is incomplete, none of those questions have defensible answers. And programs that cannot answer them will have their autonomy constrained by the same governance pressure that should be enabling it.
Start where agents can earn trust.
The answer is to be deliberate about where you start, not to slow down AI adoption.
The single biggest bottleneck in exposure management has never been finding vulnerabilities. It’s been everything that has to happen before someone can actually fix one. Who owns this asset? Is this the same finding coming from three different scanners? Which team does this route to?
In a large enterprise with incomplete CMDBs, inconsistent asset tagging, and a dozen tools that each see the environment differently, those questions consume enormous analyst time before a single remediation ticket can be created with confidence. Nearly 40 percent of organizations still rely on manual workflows for most of their vulnerability remediation process. And in most organizations, security does not own the fix. Infrastructure does. ITOPS does. The application team does.
If ownership attribution is wrong, or the same finding arrives as three duplicate tickets, or the routing logic does not match how the organization actually operates, the ticket sits. The relationship between security and infrastructure deteriorates. The vulnerability stays open.
This is exactly where agentic AI should start: inferring asset ownership from available signals, deduplicating findings across scanners, and routing work to the right person with the right context. That is low risk. The value is immediate, and solving it alone pays for the program in many organizations. If teams could focus on fixing things rather than figuring out who is supposed to fix them, remediation velocity would be unrecognizable.
Start there, prove the agents are working from a complete enough picture to get the clerical work right, then expand.

That dynamic is why so many AI-powered recommendations go unactioned. Not because practitioners do not trust AI. Because they cannot see what it knew and what it did not know when it made the call. Explainability closes that gap. It is how you move from AI-assisted decisions to AI-driven ones, with the confidence of your security team, your leadership, and your auditors behind you.
When practitioners can follow the reasoning and validate it, trust expands. When they cannot, it contracts. Over time, that trust determines how much autonomous action an organization is willing to extend. That’s not a product feature, that’s how programs earn the right to scale.

More findings never meant more security, and more AI capability on its own does not either. Faster agents operating on incomplete data is just a faster way to be wrong at scale.
What builds programs that actually reduce risk is a complete, correlated, context-rich picture of your specific environment. AI that can reason over it consistently, explain its conclusions, and support decisions that practitioners can validate and leadership can defend. And agents that earn their autonomy by demonstrating they have the full picture before they act, not after.
The goal has been the same for twenty years. We finally have the tools to actually deliver on it, but only if the foundation is right.
